If you have previously seen a health professional and would like your medical record transferred to our practice, please complete the form below.

Ideally, Health 104 would like to have your previous medical records in our system, as this will provide us with a more complete picture of your health. Information should include your medical history, past and current medications and information such as your immunisation records.

Please note that your former clinic will only forward a copy of your medical record. Legally, they are required to retain any inactive records in their system until a patient has reached the age of 25 years or for a minimum of seven years from the time of last contact;  whichever is the longest.

You may be charged a transfer fee by your previous clinic. Also, please be aware that some clinics will not process your request promptly and it may take several weeks for your medical record to be transferred.

If you would like to transfer your medical record from Health 104 to another GP clinic, then please complete a transfer form from your new practice and fax or email it to us.

A $25 transfer fee applies.

Once the fee has been paid, your file will be securely sent to the other clinic.

We try to accommodate these kinds of requests within two week of receipt.
